Referrals
Program Eligibility requires that the participant be an adult of at least 19 years of age with any one or a combination of the following events within the past two years:
- Post Heart Attack (MI)
- Post Bypass Surgery (CABG)
- Post Angioplasty (PTCA)
- Post Aortic / Mitral Valve surgery
- Cardiomyopathy
- Cardiac Transplant
- Congestive Heart Failure
- Stable CAD/angina
- Unstable angina
